(૧) મારા પુત્ર/પુત્રી/આશ્રીતની જન્મ પ્રમાણપત્ર આ સાથે સામેલ છે.(નોંધ:- જો જન્મ પ્રમાણપત્ર રજુ ન કરી શકે તો એકરારનામું અવશ્ય ભરવાનું રહેશે.)
(1) The birth certificate of my son/daughter/dependent is enclosed herewith. (Note:- If the birth certificate cannot be produced,then a declaration form must be filled.)
(૨) આથી હું ખાતરી આપું છું કે મારા પુત્ર/પુત્રી/આશ્રીતના સંબંધમાં આપેલી માહિતી મારા જાણ મુજબ ખરી છે.અને તેમાં કોઈપણ જાતનો ફેરફાર થઇ શકશે નહિ. જેની જાણ બદલ નીચે સહી કરવામાં આવેલ છે.
(2) I hereby certify that the information given in respect of my son/daughter/dependent is correct to the best of my knowledge and there can be no change in it. In recognition of which, the undersigned has signed.
